Your Role & Impact
As an Ambulatory Care Pharmacist, you’ll be a vital member of our care team, responsible for optimizing medication use and supporting chronic disease management. You’ll provide Medication Therapy Management (MTM), collaborate with providers and external agencies, and educate patients to ensure safe and effective treatment.
Your work will help improve health outcomes, reduce disparities, and strengthen our community through expert, compassionate care.
What We’re Looking For
Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited college
Current and unrestricted California Pharmacist license
Advanced Practice Pharmacist license
At least 2 years of experience as a pharmacist
Clinical pharmacy residency or equivalent experience
Strong knowledge of Medication Therapy Management and chronic condition guidelines
Proficiency with EHR systems (EPIC) and Microsoft Office

The pay range for this role is $62 - $93 on an hourly basis.Pay transparency: If you are hired at TrueCare, your salary will be determined based on factors such as education, knowledge, skills, and experience.
