Job Description

Position Overview

This role supports a caseload of approximately 50 adult participants with intellectual and developmental disabilities residing in group homes, apartments, and private residences within the Rapid City community. The LPN plays a vital role in the participant’s care team by providing medical guidance, training, and support to Direct Support Professionals (DSPs), who are non-medical personnel. This position is primarily administrative and educational, ensuring quality care planning and overseeing staff responsible for hands-on care.

Key Responsibilities

Participant Care & Medical Oversight

  • Conducts initial and ongoing assessments, including nutrition, pharmacy, plan of care, infection control, medical history, level of care certification, nursing care reports, medication, laboratory values, and psycho-social issues.
  • Addresses emergency medical needs as they arise in residential homes.
  • Provides consistent communication with medical providers to advocate for and follow up on participant health concerns.
  • Accepts and documents physician orders, ensuring all relevant parties (staff, guardians, caregivers) are informed of any new or changed interventions.
  • Performs medical procedures such as IM injections, Foley catheter insertion, supra-pubic catheter changes, and non-delegable wound care as required per certified medical professional orders.
  • Attends specialty medical appointments as needed.

Training & Support

  • Educates and trains non-medical support staff on health-related care, including medication administration certification and other medical training as required.
  • Ensures all DSPs are well-equipped to meet the health needs of participants.
  • Provides consultation and ongoing support to caregivers and guardians.

Administrative Duties

  • Maintains current and accurate medical records, including appointment logs, medical data forms, and doctor’s orders.
  • Completes and manages the agency’s Infection Control Program, maintaining logs and making safety recommendations.
  • Oversees medical scheduling and confirms upcoming appointments in collaboration with the Medical Support Assistant (MSA).
  • Utilizes EMR systems such as Therap and SetWorks for medical documentation and care planning.

Caseload & Work Environment

  • Participants range from independent individuals with jobs to those with global developmental delays.
  • Caseloads are balanced based on needs, location, and group home assignments to ensure efficiency.
  • Some participants require weekly check-ins, while others may need only a few visits per year.
  • Limited travel; complex cases may be handled through the on-site day program at the administrative building.

Current LPN license in South Dakota

Current BLS Certification
