Job Description

We are seeking a dedicated Nurse Practitioner to join a growing clinical team in Fort Wayne, IN. This role focuses on delivering high-quality, patient-centered care in a clinic setting while managing both acute and long-term medical needs. The Nurse Practitioner will work collaboratively with a Chief Medical Officer and physician partners to ensure evidence-based care, continuity, and positive patient outcomes.

Key Responsibilities

  • Perform comprehensive physical examinations and preventive health assessments
  • Order, interpret, and evaluate diagnostic tests to assess patient health needs
  • Develop and manage individualized, evidence-based treatment plans
  • Prescribe oral, parenteral, intramuscular, and subcutaneous medications, including controlled substances, in accordance with established guidelines and collaborative agreements
  • Maintain accurate and timely documentation in the electronic health record per Quality Assurance standards
  • Provide patient education on health maintenance, illness prevention, and treatment plans
  • Coordinate referrals and connect patients with appropriate community resources
  • Provide continuity of care through on-call rotation participation and telephone consultations

Required Qualifications

  • Graduate of an accredited School of Nursing (BSN required)
  • Graduate of an approved Nurse Practitioner program
  • Minimum of 3 years of professional nursing experience
  • Active Indiana Registered Nurse license
  • Advanced Practice certification/licensure
  • Active CSR and DEA license
  • CPR certification
