Job Title : Occupational Therapist (OT) - Home Health

Salary : $120,000 - $130,000

Location : San Francisco

We are looking for a dedicated and compassionate Occupational Therapist (OT) to join our home health team in San Francisco. As an OT, you will provide skilled services in collaboration with Case Managers to help individuals live safely and productively in their home environment. Your expertise will empower patients through tailored care plans addressing areas like home safety, medication management, pain management, fall prevention, and caregiver education. Key Responsibilities

• Evaluate patients' needs and develop care plans to address mobility, safety, and independence in their homes.
• Provide skilled services in home safety, wheelchair positioning, fall prevention, pain and chronic disease management, and caregiver training.
• Serve as an educator, patient advocate, and a key member of the interdisciplinary care team.
• Collaborate with Case Managers to ensure quality, comprehensive care for each patient.

Qualifications

• Current, unrestricted California Occupational Therapist License.
• BLS/CPR Certification.
• Valid California Driver’s License.
• Strong understanding of community resources, individual development, and the impact of illness/injury.
• Familiarity with state and federal home care guidelines (preferred).
• Experience in home health care and interdisciplinary team collaboration (preferred).
• Case management fundamentals and working with high-acuity patients (preferred).
• Self-directed, empathetic, and a strong patient advocate.
