Role Description

This is an on-site, full-time role for a Physical Therapist at our facility in the Springfield, Massachusetts Metropolitan Area. The Physical Therapist will assess, plan, and administer therapeutic interventions and individualized treatment plans for patients. Responsibilities include evaluating physical functions, providing rehabilitation services, and collaborating with a multidisciplinary team to improve patient outcomes. The role involves working closely with patients to maximize their mobility, relieve pain, and enhance quality of life.

Qualifications
  • Expertise in Physical Therapy, Rehabilitation techniques, and related therapeutic practices
  • Strong skills in Medicine and Patient Care, with a focus on individualized treatment plans
  • Experience in providing Home Care services and understanding of in-home therapy approaches
  • Exceptional interpersonal and communication skills to interact effectively with patients, families, and healthcare teams
  • Current Physical Therapy license in Massachusetts and compliance with state regulatory requirements
  • Bachelor’s or Doctorate degree in Physical Therapy from an accredited program
  • Ability to work on-site and travel locally to provide home-based services as needed
  • CPR certification and a valid driver’s license are preferred
